What is SSA-89

In some cases, you must use the SSA-89 form, titled “Authorization for the Social Security Administration to Release Social Security Number (SSN) Verification”, to prove that the social security number provided is yours.

This procedure is adopted so companies can protect themselves from identity theft and fraud.

It’s essential to be sure that we are dealing with the right person associated with the correct social security number.

To do that, we use the SSA 89 authorization form.

This form can be used by anyone needing to verify your social security number with a company or agency for any reason, such as:

  • If you are looking to get a mortgage 
  • You are applying for a personal loan
  • If you are applying for a license 
  • If you are in a recruitment process with an employer for background check purposes 

Typically, a company or organization wanting to verify your SSN will complete the form and ask that you sign it so they can then proceed to investigate your number.

What is the purpose of SSA 89 Forms

Using SSA89, you are authorizing Social Security to verify your social security number through a third party.

The SSN verification will be done based on a User Agreement between the SSA and the Requesting Party for Consent Based Social Security Number Verification (CBSV).

The reason why a company may enroll in the CBSV for any of the following reasons:

  • Mortgage service
  • Background check
  • Credit card
  • Bank service
  • License requirement
  • Other 

It’s important to verify your social security number to ensure that you are the right person associated with the social security number.

If a mortgage lender or bank is looking to grant a significant sum of money to someone, they may have a standard process of requiring SSA 89 forms to verify the borrower’s SSN.

This can protect mortgage providers and financial institutions from getting defrauded by individuals pretending to be someone else.

Outline of SSA Form 89

The CBSV SSA 89 form is quite simple.

It’s actually a one-page document with the following outline:

  • Your identification area
  • Reason for using CBSV
  • Company identification 
  • Consent validity period
  • Signature block

How do you fill out the SSA 89

Here, we have the SSA-89 instructions on how to fill out the form.

You’ll need to include your name, date of birth, and Social Security Number on the top part of the form.

Then, you must provide a brief summary of the nature of the business transaction that you are performing where the third party needs to verify your SSN.

How to submit SSA-89 forms

The difference between the SSA 89 form and other SSA forms is that this one will be sent to the third party you’ve identified on the form instead of the SSA office.

Essentially, you are sending the form to the company or agent of the company to verify your Social Security Number and identify.

Once the company receives your form, they can adhere to the Consent Based Social Security Number Verification agreement to get the SSN verifications they need.

Upon completion of the SSN verification, you’ll be able to advance in your business transaction whether it be the request for a mortgage or personal loan (for example).
